based chemists/ qualified Class-A Pharmacists involved in running institutional
pharmacy business/ chain of pharmacies (Owner of Medical Store and having retail drug
sale license for procurement / sales of medicines and registered with sales tax and
income tax departments) for annual contract Financial Year 2022-23 for supply of Local
Purchase of Medicines and surgical disposable to Combined Military Hospital Sialkot
on Daily Basis as well as for CNE Patients by Bill (Rebate should be clearly mentioned).

- Quotations in sealed envelope should clearly mention rebate on retail price.
Envelope labeled as “RE-TENDER FOR DAILY LP CONTRACT CMH SIALKOT FOR
FY 2022-23” should reach this office by 1100 hours on 11 Oct 2022. Bids will be
opened on 11 Oct 2022 at 1130 hours in presence of bidders / representative.
- The bid shall comprise a single package containing two separate envelopes.
Each envelope shall contain separately, the FINANCIAL PROPOSAL and the

- The procuring agency reserves tha rights to accept or reject bids according to

Rs: 5000/- (Non-refundable) in Accounts Branch office of this hospital.
